We are very excited about our summer program! We hope you see something you will be interested in to enhance your summer vacation.

    • Completed Applications (with deposit) are due by Friday, May 7, 2004.
    • Assignment Letters will be mailed out Friday, April 23, 2004.
    • The Required Forms (listed below) must be completed and returned with your payment balance by Friday, May 14, 2004.

Required Forms:
    » Completed Application with $75 non-refundable deposit - be sure to include "Statement of Interest"
    » General Permission Form
    » New Student Demographic Information Form
    » R-rated Movie & Swim Permission Form
    » School Medication Prescriber Parent Authorization Form (if applicable)
    » Most recent grade report
    » A copy of the front and back of your insurance card

Classes:
    » Week One: June 14-18. Week Two: June 21-25. Week Three: June 28-July 2
    » Monday through Friday from 8:00 am to 3:15 pm with an hour break for lunch

Check-In:
    » The Sunday before your session begins from 2:30 pm to 3:30 pm
    » Get acquainted meal that evening

Cafeteria:
    » Open each day during the week serving breakfast, lunch, and dinner
    » Field trips may take a lunch with snacks or our cafeteria will hold their dinner

Packing for ASMS:
    » Sleeping bag or sheets (twin extra-long), blanket and pillow
    » Towels, toothbrush, toothpaste, shampoo, soap, etc.
    » Comfortable clothes, shoes, swim suit
    » Pencils, pens, notebook
    » Alarm clock - you don’t want to be late for class!
    » Snacks - optional

Awards Ceremony/Check-Out:
    » Awards Ceremony at 3:30 pm, Friday, in the school auditorium. Parents Invited!
    » Check out after ceremony

Money:
    » A little change for laundry, drinks, snacks, etc.

Carpool:
    » A list of those interested will be supplied

Two-Week Session Attendees:
    » Pay $50.00 to stay over the weekend
    » Check out Friday of your last session
